1. Fill the application online and submit the properly filled application with relevant soft copies of the certified documents.
2. Application fee of 1,500/= can be paid to any branch of the People’s Bank to credit collection A/C No. 033010003532 of the University of Sri Jayewardenepura.
3. After you have made the payment of the application fee, upload the following documents in PDF formats.
   1. Certified copy of the Birth certificate
   2. Certified copy of the NIC/Passport
   3. Certified copies of the certificates of Academic/Professional qualification(s)
   4. Certified copies of the certificates of other relevant qualification(s)
   5. Bank payment receipt
